Community/Patient Resource Specialist
Job Description
The Community/Patient Resource Specialist will engage patients, create trusting relationships, while navigating Forks Community Hospital service departments, and closely communicating with the patient's entire care team based on an individualized care plan. The Community/Patient Resource Specialist will ensure social determinants of health are addressed and the patient is referred to applicable resources. This position will participate in the development of new patient care resources within the FCH organization. They will work with the Rural Health Clinics and Forks Community Hospital Case Management to develop palliative care including staff and community education coordination.
Qualifications:
High School diploma or equivalent required. 2-3 years of experience in community or health settings preferred. Must demonstrate ability to communicate effectively. Basic computer knowledge. Comfortable having crucial conversation regarding payment options and patient responsibility. Strong time management, organizational and planning skills. Commitment to impacting the care of high-risk patients.
Experience: Previous experience working in health care, case management, and/or care coordination. Project development and public speaking. B.S./B.A. in a healthcare or social services related field or 2-3 years' experience desirable.
Salary: $28.63-$44.30/hour DOE
Shift: M-F, 8am-4:30pm
