Clinical Case Manager - Acute Social Services, Per Diem
The Clinical Case Manager coordinates the organization of clinical documentation, utilization management, and coordination of care across the continuum. This position will review, assess, and identify high risk and high resource consumptive cases. The clinical case manager works collaboratively with the health care team to achieve national standards for best practices, discharge planning, and clinical documentation and utilization review management functions. The Clinical Case Manager strives for continuous quality improvement in the delivery of clinical and cost-effective health care delivered at Forks Community Hospital. These duties will be performed in accordance with and in adherence to the mission, vision and values of Forks Community Hospital. Responsible for the supervision of Social Services Case Management department at Forks Community Hospital and Clinics. Responsible for maintaining policies and procedures for Social Services and the swing bed unit.
Education/Training/Knowledge: RN, BSN preferred and/or 5 years clinical experience in the Acute care setting. License/Certification: Current Washington State RN, Case Manager Certification preferred. BLS, ACLS, PALS and NRP within six months of employment. Experience: Practical experience in discharge planning and social services, preferred. Combination of education and clinical experiences will be taken into consideration. Skills/Ability: Ability to work independently, problem solve, set priorities and flex to the workload requirements. Must have effective written and verbal communication skills. Excellent interpersonal skills; demonstrated written and verbal communication skills. Demonstrates accountability for position. Must continuously demonstrate the ability to be an effective team member and educator. Ability to deal positively in stressful situations and elicit cooperation inter departmentally. Demonstrate an ongoing ability to create a sensitive and accepting climate for patients, families and coworkers. Computer skills required. Windows proficiency strongly desired. Physical Demands: Must be able to stand, walk and sit for varied lengths of time. Must be able to bend in order to access documents in file cabinet.
Weekdays, 7:00 am - 3:30 pm
SALARY: $39.17 - $54.82/hour DOE
